Perry Public Library is an official Passport Acceptance Facility for the U.S. Department of State. Please call (440) 259-3300 for more information or to make an appointment.
We are by appointment only.
We do not offer photo services.
If you are renewing your passport, be sure to read over the requirements. We provide forms for passport renewals (Form DS-82); however, you are responsible for mailing your renewal application or you may renew online if you are eligible.
Our passport service hours are Monday-Thursday from 9:00 AM to 7:00 PM, Friday from 9:00 AM to 4:00 PM, and Saturday from 11:00 AM to 2:00 PM.
How to Apply
If this is your first passport, you must apply in person. You need:
- A completed, unsigned passport application (Form DS-11) filled out in black ink
- Proof of U.S. citizenship (most commonly a U.S. birth certificate or a Certificate of Naturalization or Citizenship)
- Current proof of identity (most commonly a fully-valid driver's license or enhanced driver's license with photo)
- One passport photo
- A check or money order made out to the U.S. Dept. of State (see below under “Fees”).
- $35 facility acceptance fee
*Additional documentation is required for minors under the age of 16.
Fees
A check or money order must be paid directly to the U.S. Department of State and an execution fee is paid directly to Perry Public Library. Please make sure you bring two forms of payment: a check or money order is required for the U.S. Department of State fee. The Execution Fee of $35.00 per application paid to Perry Public Library can be paid with cash, check, or credit card.
